Program 01

Community Habilitation

Skills for everyday independence.

Personalized support at home or in the community — wherever the person feels most comfortable.

  • Daily living skills (personal care, cooking, cleaning)

Program 02

Day Habilitation (Without Walls)

Community-based, community-shaped.

Helping adults learn new skills, maintain existing ones, and stay actively involved in their communities.

  • Skill-building in communication and self-advocacy
  • Community exploration and participation
  • Volunteer opportunities
  • Navigating public transportation and community spaces

Program 03

Respite Services

Rest for families, joy for individuals.

Structured relief for families and engaging experiences for participants.

  • In-Home Respite: personalized support in the family's home
  • After-School Respite: weekdays 3–6 PM year-round
  • School Holiday Respite: structured programs, ages 6–17

Program 04

Employment Services

Meaningful careers, real confidence.

A coordinated, step-by-step process that leads to competitive community employment.

  • Pre-Vocational Services: workplace habits and soft skills
  • ACCES-VR: career exploration, planning, and job placement
  • Supported Employment (SEMP): job coaching and on-the-job support

Program 05

Housing Subsidy

Safe, affordable, self-directed.

Financial assistance so individuals can live in the communities they choose.

  • Financial assistance for rent and utilities
  • Flexibility to live alone, with roommates, or with a caregiver
  • Collaboration with Care Managers and OPWDD

Program 06

Broker Services & Self-Direction

You direct your own supports.

Person-centered planning with knowledgeable Support Brokers.

  • Person-centered planning and goal development
  • Budget planning and resource management
  • Assistance hiring and supervising support staff
  • Coordination with Care Managers
